8. Please revise Section 6.4.4(e)(i) to indicate the Division of Reclamation, Mining and Safety instead <br />of the Division of Mining and Geology. <br />9. In Section 3.1.13, the applicant refers to the Division as the Division of Minerals and Geology and <br />DMG. Please revise the Spill Reporting section to indicate the Division of Reclamation, Mining and <br />Safety instead of the Division of Minerals and Geology. <br />6.4.5 Exhibit E - Reclamation Plan <br />10. The applicant states the landowners do not want the access/haul road reclaimed. Please provide a <br />notarized letter from the landowners, Len L. and Roxie L. Camp, to this affect and indicated the <br />access/haul road(s) on the Mining Plan and Reclamation Plan Maps. <br />6.4.6 Exhibit F - Reclamation Plan Map <br />11. Please explain why Kimberly A. and Charles A. Reid are indicated as property owners within the <br />permit boundary on the Reclamation Plan Map and revise all applicable exhibits accordingly. <br />6.4.12 Exhibit L - Reclamation Costs <br />12. The Division prepared a reclamation cost estimate for the Leone Huerfano Gravel Pit. The estimate <br />was based on a phased mining operation, where only 22.5 acres will be affected at any given time. <br />The Division estimates the total cost to reclaim the site is $61,000.00. A copy of the estimate is <br />enclosed for review. <br />6.4.13 Exhibit M - Other Permits and Licenses <br />13. Please commit to supplying a copy of all other permits and licenses to the Division, including but <br />not limited to; Huerfano County Conditional Use Permit, Colorado Air Pollution Control Division <br />Fugitive Dust Permit and Air Pollution Emission Notice (APEN), Water Quality Control Commission <br />Stormwater Discharge Permit, all required Army Corps of Engineers permits, and Colorado <br />Department of Public Health and Environment Storm Water Permit. <br />6.4.18 Exhibit R - Proof of Filing with County Clerk and Recorder <br />14. Please provide an affidavit or receipt indicating the date on which the revised application information <br />indicated above was placed with the local County Clerk and Recorder. <br />6.4.19 Exhibit S - Permanent Man-made Structures <br />15. The applicant submitted the San Isabel Electric Association, Inc. - Easement I - Exhibit A depicting a <br />relocated power line location along CR 611.1. Please provide the Division with additional <br />information pertaining to the timing, duration and affect on mining and reclamation activities of <br />the power line relocation.
