This letter is to inform you that your company is the apparent low bidder on Project PKA -9 -1149, the Fruita Mine
Maintenance Bond Revocation Project. To complete the process of bid award, the following information must be
received by me in the offices of the Division of Reclamation, Mining and Safety as soon as possible:
1) A certificate of insurance and Additional Insured Endorsement, in the amounts given in Article 3,
INSURANCE, of the General Conditions. The certificate of insurance must show the State of
Colorado as an additional insured. Also, please make sure that the Certificate of Liability
Insurance has a cancellation policy of 45 days notice.
2) A performance bond and a labor and materials payment bond in the amounts of 100 percent
and 100 percent respectively of the bid amount ($23,560.00). The bonds must be submitted
using the State forms. Copies are enclosed. NOTE: Please inform your Bonding Company to
leave the date on the first page of each bond blank. The date will be entered by the
Natural Resources Purchasing Office.
3) A project schedule must be submitted, as specified in Article 5 PROJECT SCHEDULE of the
General Conditions.
4) A completed AML Contractor Ownership and Control Information Package.
5) For contractors new to the state, or contractors who have not worked for the state recently, a
w -9 form is attached.
6) Also included is a Contractors Daily Site Report. This must be submitted with your invoices, or
we will not be able to make the payment, per Article 11 of the General Conditions.
When the above documents have been submitted and found to be in order, and the contract agreement fully -
executed, the Notice to Proceed will be given. If you have any questions, please call me at (303) 866 -3567, ext.
