Cattany <br />Division Director <br />Natural Resource Trustee <br />This letter is to inform you that your company is the apparent low bidder on Project PKA -9 -1149, the Fruita Mine <br />Maintenance Bond Revocation Project. To complete the process of bid award, the following information must be <br />received by me in the offices of the Division of Reclamation, Mining and Safety as soon as possible: <br />1) A certificate of insurance and Additional Insured Endorsement, in the amounts given in Article 3, <br />INSURANCE, of the General Conditions. The certificate of insurance must show the State of <br />Colorado as an additional insured. Also, please make sure that the Certificate of Liability <br />Insurance has a cancellation policy of 45 days notice. <br />2) A performance bond and a labor and materials payment bond in the amounts of 100 percent <br />and 100 percent respectively of the bid amount ($23,560.00). The bonds must be submitted <br />using the State forms. Copies are enclosed. NOTE: Please inform your Bonding Company to <br />leave the date on the first page of each bond blank. The date will be entered by the <br />Natural Resources Purchasing Office. <br />3) A project schedule must be submitted, as specified in Article 5 PROJECT SCHEDULE of the <br />General Conditions. <br />4) A completed AML Contractor Ownership and Control Information Package. <br />5) For contractors new to the state, or contractors who have not worked for the state recently, a <br />w -9 form is attached. <br />6) Also included is a Contractors Daily Site Report. This must be submitted with your invoices, or <br />we will not be able to make the payment, per Article 11 of the General Conditions. <br />When the above documents have been submitted and found to be in order, and the contract agreement fully - <br />executed, the Notice to Proceed will be given.
