Evergreen AnaIyticaI, I nc. <br />Date: 11-Nov-08 <br />Client Project ID: MMW #1 <br />Lab Order: 08 -7641 <br />SAMPLE RECEIVING <br />CASE NARRATIVE <br />Sample(s) were hand delivered to the laboratory by the client. <br />Custody seals were present and intact. <br />The temperature of the sample(s) upon arrival was 2.0 °C. <br />Sample(s) were received in good condition, in the proper container, and within holding times. <br />Sample(s) were preserved properly. NJO <br />QUALITY ASSURANCE (QA) <br />Analyses performed on samples in this work order by EAL meet- the requirements of the EAL Quality <br />Assurance Program unless otherwise explained. Analyses of RCRA samples meet the requirements of <br />NELAC and Utah Rule R444 -14 unless otherwise explained. TP <br />CLIENT SERVICES <br />Copper analyte was added to the total metal test per Tom Cavanaugh`s request on 11/11108. There are <br />no other anomalies to report. TP/PM <br />GENERAL CHEMISTRY <br />There are no anomalies to report. MM <br />METALS ANALYSIS <br />Barium at 0.004 mg/L, Manganese at 0.007 mg/L, Potassium at 0.57 mg/L, and Sodium at 3.68 mg/L <br />was detected in the method blank (MB). These amounts were not subtracted from the sample results. <br />The matrix spikes (MS/MSD) recoveries for several analytes are outside the QC limits possibly due to <br />matrix interference, since the laboratory control spike (LCS) recoveries is within the QC limits, proving <br />the analysis is in control.
