• OTHER PERMITS AND LICENSES REQUIRED EXHIBIT M <br />The following additional permits are necessary for this site: <br />1. CDPHE Combined Process and Stormwater Permit <br />This permit has been submitted for the site. It will require a revision due to the extensive changes <br />required as part of the County and City of Rifle Permitting Process. A. new application has been <br />submitted to CDPHE. It is enclosed in Appendix A. <br />2. Air Emissions Permit <br />A revised Air Pollution Emission Notice (APEN) for a Fugitive Dust Permit for the site is <br />included in Appendix E. All portable plants will have their own emissions permits. <br />3. USACOE 404 Permit <br />The current plan avoids most wetland. The delineation for this operation is approved, with the <br />removal of some wetlands under the Tulloch Rule. A 404 permit will not be needed for this <br />operation. <br />4. CDOT Access Permit <br />A permit has been approved for the site, but must be re-approved due to the time that has elapsed <br />since the original application. A maximum of 335 trips are allowed for the access on the frontage <br />road. A condition of this permit is the paving of the road from the Lafarge Mamm Creek <br />entrance to the future entrance of the pit. <br />5. City of Rifle Watershed Permit <br />This application is currently pending. Changes brought about by the Garfield County permitting <br />process to the site plan, as seen in this permit, have been integrated into the Watershed Permit. <br />6. County Land Use Change Permit <br />This permit was approved on March 15, 2010. Technical Revision 01 or this DRMS permit <br />integrates the changes from the county permit into the DRMS permit. <br />Scott Pit March 10 H-1
