(Page 2) <br />MINE ID # OR PROSPECTING ID #: m 1999-0.58_ <br />INSPECTION DATE: 2/1ni1n INSPECTORS INITIALS: RJR <br />OBSERVATIONS <br />The Division conducted a joint monitoring inspection of the site with a representative <br />from BLM, ON 2/10/10. The site is a small 9.9 acre 110 silica/white mica mine located <br />approximately 20 miles West of Canon City along the north side of High way 50. <br />Present at the time of the inspection were Miss Stephanie Carter from BLM and Mr. Aaron <br />Tezak, the operator. <br />Site was not active at the time of the inspection. When the site was last inspected in <br />2001, at the request of the operator BLM calculated the financial warranty. At that time <br />the warranty was calculated at $10,200. The operator was requested to post the additional <br />warranty in the amount of $6,000.00 which was never posted. The current warranty held by <br />the Division is 4,200. <br />The entire affected area was inspected, and it appears very little has changed since the <br />last inspection. According to Arron, due to slope being unstable, they were having <br />difficulty cutting the benches, which was noted at the time of the inspection. The north <br />eastern portion of the slopes was showing signs of failure surface. Arron was informed <br />that if there is difficulty in maintaining the benches, the operator can submit a permit <br />revision to change the current approved mining and reclamation plan. He said he will <br />consider that after receiving the inspection report. <br />As stated the site was not active at the time of the inspection. A small stockpile of <br />processed material was noted along the south side of the pit area. Benches along the <br />northwest side appeared more stable the north east side. <br />A screen and grizzly and old truck were parked along the east and south side of the <br />permit area. The drainage control along the road was found to be in good repair. The <br />total affected area did not change much from what was noted during the 201 inspection. <br />However since the operator never posted the additional warranty as requested in 2001, the <br />Division recalculated the warranty for the site and the calculation are attached with the <br />inspection report. <br />According to the new warranty calculation for the site, the operator will need to post an <br />additional warranty in the amount of $11,447.00 for a total warranty of $15,647.00. The <br />additional warranty must be posted within 60 days of receiving this inspection report.
