Dear Mr. Settle:
On March 05, 2010, the Division of Reclamation, Mining and Safety received your 112 Construction Materials Reclamation
Permit Application. package for the East Bradbury Pit, File No. M-2010-016. Preliminary review of the information received
determined that the following additional items must be received before the Division can consider your application
submitted, and technical review can begin:
APPLICATION FORM:
EXHIBIT A - Legal Description (Rule 6.4.1): The legal description must identify the affected land and be wholly
adequate for field location of the property.
Note: The affected area must include the proposed new haul road.
(1) (b) location of the main entrance to the site reported as latitude and longitude
EXHIBIT C - Pre-mining and Mining Plan Map(s) of Affected Lands (Rule 6.4.3): One or more maps may be
necessary to legibly portray the following information:
Note: The maps provided need to extend details (topography, structures, landowner, etc) further south to include
areas within at least 200 feet of the affected land.
(b) the name and location of all creeks, roads, buildings, oil and gas wells and lines, and power and
communication lines on the area of affected land and within two hundred (200) feet of all
boundaries of such area;
