Preliminary review of the information received <br />determined that the following additional items must be received before the Division can consider your application <br />submitted, and technical review can begin: <br />APPLICATION FORM: <br />EXHIBIT A - Legal Description (Rule 6.4.1): The legal description must identify the affected land and be wholly <br />adequate for field location of the property. <br />Note: The affected area must include the proposed new haul road. <br />(1) (b) location of the main entrance to the site reported as latitude and longitude <br />EXHIBIT C - Pre-mining and Mining Plan Map(s) of Affected Lands (Rule 6.4.3): One or more maps may be <br />necessary to legibly portray the following information: <br />Note: The maps provided need to extend details (topography, structures, landowner, etc) further south to include <br />areas within at least 200 feet of the affected land. <br />(b) the name and location of all creeks, roads, buildings, oil and gas wells and lines, and power and <br />communication lines on the area of affected land and within two hundred (200) feet of all <br />boundaries of such area; <br />Office of Denver - Grand Junction - Durango Office of <br />Mined Land Reclamation Active and Inactive Mines
