(Page 4) <br />MINE ID # OR PROSPECTING ID #: M-1277-151 <br />INSPECTION DATE: -1/16/1 n INSPECTOR=S INITIALS: MAC- <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when considered <br />by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. The Reclamation Plan map does not reflect the current configuration of the ponds. Rule 4 / 16 /10 <br />6.4.6(a) states the Reclamation Plan Map must show the expected physical appearance <br />of the area of the affected land. <br />The Operator shall submit a Technical Revision to the Division. The Technical Revision shall <br />describe the changes to the Reclamation Plan as they relate to the final configuration of the <br />ponds and shall include an update Reclamation Plan Map. The Technical Revision shall be <br />submitted to the Division on or before April 16, 2010. <br />2. <br />The Greeley Pit contains approximately 68 acres of exposed groundwater. The Operator <br />does not have a well permit and substitute water supply plan or court approved <br />augmentation plan. Rule 3.1.6(l) (a) requires Operators to comply with applicable <br />Colorado water laws and regulations governing injury to existing water rights. <br />The Operator shall obtain a well permit and temporary substitute water supply plan from the <br />Division of Water Resources.
