Tim Haynes, Environmental Engineer <br />2. 19302 County Road 3, Parshall, CO 804568 <br />3. phone: 303-569-3221 ext. 2284 <br />4. fax: 970-725-0038 <br />(d) Samples were not necessary to evaluate the situation or corrective actions <br />(e) After separating the flange which was determined to be leaking, the personnel <br />working on the issue noticed that the gasket had become nearly severed at the <br />six 'o'clock position. Engineering suspected that the gasket simply failed or may <br />have been compromised during the original installation. There was no indication <br />on the flange facing that channeling had occurred (indicative of a prolonged <br />passage of water). After the flange faces were inspected and found to be in <br />satisfactory condition, a new gasket was placed and the flanged connection was <br />reconnected. These repairs were all completed by February 10`h, 2010. The line <br />was recharged and no leaks were visible. After burying the line, the Tailing & <br />Transportation group will continue to monitor the area to ensure the condition <br />does not occur again.
