6.3.1 Exhibit A - Legal Description and Location Map <br />3. In accordance with Rule 6.3.1(3), please revise the Location Map to show the names of all <br />immediately adjacent surface owners of record. <br />6.3.2 Exhibit B - Site Description <br />No comment <br />6.3.3 Exhibit C - Mining Plan <br />4. The Applicant has stated 15 feet of topsoil will be removed during mining. The Division <br />defines topsoil as material at the surface of the earth which has been so modified and acted <br />upon by physical, chemical, and biological agents that it will support rooted plants necessary <br />to achieve reclamation goals. The soil survey from the Natural Resources Conservation <br />Service indicates that there at least 12 inches of topsoil present at the site. Please commit to <br />salvaging and replacing topsoil to a depth of 6-12 inches. <br />5. Please state how long topsoil will be stockpiled. If topsoil is to be stockpiled longer than one <br />year, then a vegetative cover must be used to stabilize the stockpile until it is used in <br />reclamation. The Applicant shall indicate the type of grass species to be planted and the <br />applicable quantities. Specify the quantity of each grass species as pounds of pure live seed <br />per acre. <br />6. The Applicant stated the existing mining area is 4,885 square feet. Please specify the depth of <br />the excavation and the grade of any highwalls. Also, indicate if any bench work has been <br />done at the site. <br />7. In accordance with Rule 6.3.3(1), please describe how storm water runoff will be managed. If <br />storm water runoff leaves the site, then the Applicant must describe how storm water will be <br />protected from pollution including sediment containment systems and storm water runoff <br />controls. In addition, if storm water is discharged from the mine site, then the Applicant must <br />apply for a storm water discharge permit from the Colorado Department of Public Health and <br />Environment. <br />8. The Applicant has stated the alabaster will be `coved' on site. Please describe the nature of <br />the coving process. <br />9. In accordance with Rule 6.3.3(n), identify the primary and secondary commodities to be <br />mined/extracted and describe the intended use. <br />6.3.4 Exhibit D - Reclamation Plan <br />10. As required by Rule 6.3.4(1)(a), specify the point in the mining plan and to what depths <br />overburden will be replaced in relation to ongoing extraction.
