Sketches shall also show decking, if holes are decked to achieve different delay <br />times with a hole <br />• Diameter and depth of the holes <br />• Types of explosives used <br />• Total weight of explosives used per hole and maximum weight of explosives used per 8 <br />millisecond period; <br />• Initiation system; <br />• Type and length of stemming <br />• Mats or other protection used <br />• Type of delay detonator and delay periods used; <br />• Number of persons in the blasting crew; and <br />• Seismographic records, where required, including; <br />• Type of instrument, sensitivity, and calibration signal of the gain setting or <br />certification of annual calibration; <br />• Exact location of instrument and the date, time and its distance from the blast; <br />• Names of the person and firm taking the readings; <br />• Name of the person and firm analyzing the seismographic record; <br />• The vibration level recorded. <br />Wildcat will conduct a pre -blast survey of potentially affected structures. Pre -blast surveys will <br />be conducted at all structures located within 1 /2 mile of the proposed blast unless persons in <br />ownership of control deny access to the structures. Written request to conduct pre -blast surveys <br />will be sent to owners of all structures a minimum of 30 days prior to initiation of blasting. A <br />• blasting schedule may be used in lieu of notifying for each individual blast. Wildcat Mining <br />Corporation will submit a pre -blast survey plan for Division review and approval prior to <br />conducting the survey. <br />(p) Emergency Response Plan <br />An Emergency Response Plan has been prepared to meet Mine Safety Health Administration <br />regulations. A copy is presented in Attachment C -8. In addition Wildcat Mining Corporation will <br />comply with the following Spill Reporting requirements (Rule 3 3.1.13) <br />• Will notify DRMS of a spill of any toxic or hazardous substances, including spills of <br />petroleum products that occurs within the mined land permit area and that required <br />by CDPHE, the National Response Center, the Colorado Emergency Planning <br />Commission, local Emergency Planning Committee or the State Oil Inspector. <br />Within 24 hours of the time of the spill notice DRMS via phone, facsimile or e- <br />mail and include in the notice any relevant information known at the time of the <br />spill including Operation name, DRMS permits number and name of the person <br />reporting the spill, Telephone number of a responsible company official for the <br />Office staff, Date and time of the spill, Type of material spilled (CAS number <br />from MSDA data sheets) Estimate of the amount spilled whether any material <br />has left the permit areas and where the spilled material went, and Initial measure <br />taken to contain and clean up the spill <br />Copy of correspondence and or written reports provided other agencies per rule <br />3.1.13.(2) <br />.
