6.3.5 - Exhibit E, Map <br />(1) The Mine and Reclamation Maps need to reflect the project proposed. Inclusion <br />of "future mining area" is confusing and needs to be removed from the maps. In <br />general the map size should equal that of Exhibit A-2 so that details are easier to <br />distinguish. <br />(2) Mine Plan Map <br />a) adequate. <br />b) adequate. <br />c) Please identify all mining related features as required. <br />d) adequate. <br />e) adequate. <br />f) No previous disturbances are noted. There is an existing pond on site noted. <br />(3) Reclamation Map <br />a) Please provide elevations for contour lines and gradient for flat areas. <br />b) Not applicable. <br />c) Please note shallow areas for wildlife use if any. <br />d) It is noted that slopes will be reestablished at 2:1 until final reclamation of 3:1. <br />e) Final topsoil depth is stated at 6 to 10 inches. <br />6.3.6 - Exhibit F. Other Permits <br />As noted above, a copy of the well permit application is required. No exposure of <br />ground water can occur until the well permit is issued and proof submitted to the <br />Division. <br />6 3 7 - Exhibit G Source of Legal Right to Enter appears to be acceptable <br />6 3 8 - Exhibit H Municipalities within 2 miles: The City of Craig is noted. <br />6 3 9 - Exhibit I Proof of Filing with County Clerk is acceptable. <br />6 3 10 - Exhibit J Proof of Notice to County Board and Soil Conservation is acceptable. <br />6.4.11 - Exhibit K Proof of government Contract - N/A <br />6.4.12 - Exhibit L Permanent Man-made structures are identified. <br />• The applicant must provide a notarized letter damage agreement from the power <br />company as required under Subsection (c) prior to any decision on the application.
