A COPY OF THE VIIR11Ti N NOTICE OR ACKNOWLEDGEMENT SHALL BE AVAILABLE AT THE DRILLING SITE

1) Notice was provided talm State Engineer at least 3 days prior to construction of monitoring a observation hole(s).
2) Construction of the hole(s) must be completed Wthin Or days of the date notice was given to the State Engineer. Testing and/or
pumping shelf not exceed a total of 200 hours unless priorwritim approval Is obtained from the Ststa Engineer. Water dhrerted during testing
ghill not be used for beneficial purposes. The owner of the hole(s) is responsible far obtaining pemtlt(s) and complying with air niles and
regulations pertaining to the discharge of tlulds produced during testing.
3) All work mtmt carnply vrith the Viloter Wall Construdlon Rules, 2 CCR 402-2. Minimum construction standards must be met w a variance
obtained. Standard permit application and work report forms are found on the 1511118 wrebak at hna:/hvwa.aer?rstate.ear?c. Well
Construadon and Test Reports (GNYS-31) must be completed for each hole drilled. The licensed contractor or authorized individual must
submit the completed forms to this alfiloe within 60 days of mongorhtg hole campletlon.
4) Unless a wall patmlt Is obtained, the hoW*l must be plugged and sealed within one (1) year after construction. An Absndomnent
Report (form 011110-9) must be submitted within 80 days of plugging & sealing. The above MH edt oWedgement number, owner's
structural name, and owners nerve and address must be provided on allwall permit poication(s), well construction and abandonment reports.
6) The owner of the hote(a) phdi makrtliln records of water quality testing and submit tHa data to the State Engineer upon request
