(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1998-054 <br />INSPECTION DATE: 02/17/2010 INSPECTOR'S INITIALS: THM <br />OBSERVATIONS <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division <br />of Reclamation, Mining and Safety. The East Douglas Creek mine site is located 20 miles south of Rangely, <br />Colorado off of RBC Road No. 27. The site is a 112c operation. Stan Rasmussen and Clint Chappell both <br />representing Rio Blanco County accompanied the inspector(s) during the inspection. <br />A sign was observed at both entrances to the mine site but both signs are not compliant with Rule 3.1.12(1). <br />According to Rule 3.1.12(1) the following must be visible on the sign: The name of the operator; a statement <br />that a reclamation permit for the operation has been issued by the Colorado Mined Land Reclamation Board; <br />and the permit number. Please post legible signs at the site that meet the criteria of Rule 3.1.12 and provide <br />photo-documentation of its placement to this office no later than the date indicated on the last page of this <br />report. This is not seen as a problem but the signs need to have all information required in Rule 3.1.12(1). <br />The permit boundaries are comprised of fence posts that are easily identifiable around the entire perimeter of <br />the mine. <br />The mine is active and a loader was on site during the inspection. Various stockpiles of construction material <br />were observed during the inspection. Due to the amount of snow present at the site an assessment of the <br />revegetative success was not possible. At the east side of the mine various stormwater diversions and <br />management structures were observed. All stormwater management structures observed appeared to be in <br />properly functioning condition. Rio Blanco County staff indicated that blasting was required to mine the west <br />mine face due to it being primarily solid rock. A review of the mine plan submitted in 2004 does not indicate <br />that the use of blasting would be implemented at the East Douglas Creek mine site. This is noted as a problem <br />and a technical revision must be submitted to add the use of explosives to the mine plan or the operator <br />must stop using explosives to conduct material extraction at the site. If the operator chooses to continue to <br />use explosives at the site the operator must demonstrate pursuant to Subsection 6.5.4, Geotechnical Stability <br />Exhibit, that offsite areas will not be adversely affected by blasting. <br />No evidence of erosion or off site damage was observed during the inspection. <br />Photos that show the conditions observed during the inspection are included at the end of this report. <br />Responses to this inspection report should be directed to Travis Marshall at the Division of Reclamation, <br />Mining and Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, Grand Junction, Colorado <br />81501, phone no. 970-241-2042. <br />I & E Contact Address cc: <br />NAME: David P.
