Stormwater Inspection Report <br />Permittee: Asphalt Paving Company (APC) Cert. No.: COR34 0544 Date: 02/02/2010 <br />Facility: Ralston Quarry Industrial Type: Sand & Gravel Receiving Water: Van Bibber Creek <br />Facility Address: 6569 State Highway 93, Golden, Jefferson County, Colorado 80403 <br />Persons present: Mark Miller (Safety Manager - APC); Jeffrey Keller (President - APC); Kathy Rosow, Maura McGovern <br />(CDPHE - Water Quality Control Division <br />Facility Representative(s)/Title(s): Mark Miller (Safety Manager) Inspector(s): Kathy Rosow, Maura McGovern <br /> CDPHE (Water Quality Control Division <br />Inspection Findings <br />The Water Quality Control Division (Division) inspectors held a closing conference at the conclusion of the inspection. <br />During the closing conference, the inspectors reviewed the inspection findings with the Facility Representative, and directed <br />the representative to correct all findings. Pursuant to all provisions of the Colorado Discharge Permit System General Permit <br />for Stormwater Discharges Associated with Sand and Gravel Mining and Processing (the stormwater discharge permit), the <br />findings listed below must be corrected. <br />Compliance assistance materials provided to the facility representative at the time of the inspection include: (1) Change of <br />Contact and/or Designation of Duly Authorized Agents(s) form, and (2) Appendix B - Stormwater Management Plan <br />SWMP requirements and guidance document. <br />Records Review <br />Note: In a communication with APC prior to the inspection, the Division requested that a copy of the Stormwater <br />Management Plan (SWMP) be provided to Division personnel at the inspection. A copy of the SWMP was provided to <br />Division personnel on February 2, 2010. <br />Note: The permit certification effective date was January 22, 1993. <br />1. A copy of the Stormwater Management Plan was retained on site. The SWMP was reviewed during the inspection and <br />found to be inadequate for the following reasons: <br />a. The Site Man section in the SWMP did not identify all items required by the stormwater discharge permit. <br />Specifically, the following items were not identified on the SWMP site map, but were present at the site: <br />i. stormwater outfalls (ix.:, culvert outlet from pond 3); <br />ii. non-structural BMPs (i.e., vegetative cover implemented on some perimeter berms and slopes at the site); <br />iii. areas used for recycling of asphalt and concrete; <br />iv. all areas of soil disturbance; <br />V. mobile fueling locations; and <br />vi. areas used for mineral mining and processing (i.e., crusher and screening operations). <br />The SWMP site map must be updated to include all items required by Part I.B.2.a of the stormwater discharge permit. <br />b. The Description of Potential Pollutant Sources/Material Inventory section in the SWMP did not adequately address . <br />all pollutant sources present at the site. Specifically, this section did not identify the following pollutant sources: <br />loading and unloading operations; crushing and screening facilities; and haul roads. The SWMP must be updated to <br />include this information as required by Parts I.B.2.b of the stormwater discharge permit.
