(Page 2) <br />MINE ID # OR PROSPECTING ID #: KA-1977-1 A3.sg <br />INSPECTION DATE: 2/17i1n INSPECTOR=S INITIALS: MAC <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br />(Division). Kenneth Morrison of Lincoln County was also present for the inspection. The Fisher Pit is located <br />approximately 10 miles north of Holyoke, Colorado. The site is permitted for 36.90 acres and the post mining <br />land use is rangeland. <br />The permit sign was posted at the site entrance and the boundary of the affected area was clearly delineated per <br />Rule 3.1.12. Mining at the Fisher Pit is complete. According to the 2009 Annual Report, the Fisher Pit has been <br />inoperative for the past two years. Please note that Rule 3.1.3 requires reclamation to be completed within five <br />(5) years from the date the Operator informs the Division that mining is complete. <br />The Fisher Pit is dominated by a southerly aspect; the pit slopes have been graded to 3H:1 V. Vegetation has <br />been established on the majority of the site. Several acres in the northern portion of the site, at the top of the pit <br />slopes, are bare and do not have vegetation established. These areas may need to be reseeded in order to <br />establish vegetation. The Operator indicated that topsoil may be imported prior to reseeding. If topsoil is <br />imported, the Operator will need to provide the Division with notice of any proposed backfill activity not <br />identified in the approved reclamation plan. The Notice to the Division shall include the following: <br />a) A narrative that describes the approximate location of the proposed activity <br />b) The approximate volume of inert material to be backfilled <br />c) A signed affidavit certifying the material is inert, as defined in Rule 1.1(20) <br />d) The approximate dates the proposed activity will commence and end <br />A haul road bisects the northern portion of the site, running from west to east. The haul road connects County <br />Road 28, on the west side of Fisher Pit, to the Hoffman Pit (M-2005-044) located on the east side of the Fisher <br />Pit. The haul road will remain after completion of reclamation and will continue to be utilized by the Hoffman <br />Pit. Several acres north of the haul road require grading and seeding. Minor erosion in the form of rills is <br />occurring in this portion of the site. This has been cited as a problem and will require corrective action by the <br />Operator. Please see Page 3 of this report for the required corrective action. <br />Several corrugated metal pipes and piles of concrete were observed on the site. According to the Operator, this <br />material was likely illegally dumped at the site. This material will need to be removed and its presence has been <br />cited as a problem.
