Cattany <br />Division Director <br />Natural Resource Trustee <br />On February 10, 2010 the Division of Reclamation Mining and Safety (Division) received additional information for <br />a 110c application for the above referenced site. Please note that the information submitted was out of order and <br />mixed up. Staff had to spend considerable time sorting through the packet to ensure that the information was in <br />the correct place and order prior to any additional review. Any errors or missed information is the responsibility of <br />the applicant or the consultant. Therefore, it is incumbent upon them to ensure all information submitted to the <br />Division is in an orderly manner or bare the consequences of not having it in the intended sequence. A <br />completeness review of the application notes issues that must be corrected in order for it to be considered <br />complete for review. Please submit or correct the following: <br />1) As previously noted: Page 7of the application has no date to accompany the signature of the applicant. <br />Please submit a new, completed signature page that includes all matching dates, signatures in blue ink <br />and a new notarization. <br />2) Per Rule 1.4.2(2) (b) (iii) and 1.6.2(1) (b) a signed affidavit of notice posting is required. The affidavit <br />submitted is incomplete with no contact information noted for the applicant or County Clerk. Please <br />submit the required fully filled out affidavit. <br />3) Exhibit K is incorrect. The exhibit submitted is for Rule 6.4.11 not Rule 6.3.11. Please clarify. <br />4) Exhibit L notes an agreement with the power company or a stability study will be submitted prior to <br />mining within 200 feet. While not a completeness issue staff cannot approve the permit application <br />without one or the other.
