A signed notarized original and <br />one copy of the pages on the application form that needed to be corrected on the original <br />application for a 112 Reclamation Permit. The following items have been included: <br />1. Page one of the application now has the applicant/operator name to be used on <br />the permit. "LEONE SAND & GRAVEL LLC" please see the enclosed <br />Certificate of Good Standing from the Colorado Secretary of State. <br />2. The applicant/operator has provided documentation showing the ARTICLES <br />OF ORGANIZATION recorded with the Colorado Secretary of State. An <br />Amendment to the Operating Agreement is also included in this <br />correspondence. The modification shows the ownership of LEONE SAND & <br />GRAVEL LLC. <br />3. The REGISTERED AGENT as noted on the Colorado Secretary of State web <br />site Hope Brunelli is the Certified Public Account (CPA) for the Limited <br />Liability Company LEONE SAND & GRAVEL LLC. The signature page <br />(Page 8) of the application form has been redone. The notary section has been <br />revised; please note that the Notary Public "Carol A. Brandolino" did impress <br />her Notary Stamp at mid page right side page 8.
