CO 81623 <br />To Whom It May Concern: <br />This letter is to verify that Colorado Stone Quarries, Inc. is a member in good standing of the San Juan Mine <br />Rescue Cooperative. The Cooperative covers all mines in this area owned or operated by Colorado Stone <br />Quarries, Inc. <br />The mine rescue station for the San Juan Mine Rescue Cooperative is located at the county road maintenance <br />facility approximately one mile north of Ridgway, Colorado. This facility provides a proper storage <br />environment to assure equipment readiness for immediate use. All necessary equipment and maintenance <br />procedures as required by CFR 30. Part 49.6 are in place. <br />The San Jan Mine Rescue Cooperative consists of at least two mine rescue teams; consisting of five members <br />and one alternate, who are fully qualified, trained, and equipped for providing emergency mine rescue service, <br />as described by CFR 30, Part 492(b). <br />Please note that any team members supplied by your company must participate in refresher training courses <br />every two months, which includes mandatory wearing and use of the breathing apparatus. If one of these <br />sessions is missed, that person is no longer considered eligible by MSHA to be a team member until training is <br />received to make up for the time missed. This could possibly force closure of all mines in the Cooperative until <br />said training is completed. <br />[_t is your responsibility to provide the Cooperative with accurate, up-to-date maps of any mines covered. As we <br />are currently updating our files, please provide us with a current mine map at your earliest convenience.
