On January 26, 2010 the Division of Reclamation Mining and Safety (Division) received a 110c application for the
above referenced site. A completeness review of the application notes issues that must be corrected in order for it
to be considered complete for review. Please submit or correct the following:
1) On Page 4 of the application Items 12 and 13 have two land uses marked. It clearly states to mark one.
Please submit a correct page that identifies the primary present and post mine land uses.
2) On Page 7of the application there is no date to accompany the signature of the applicant. Please submit a
complete signature page.
3) There are multiple Exhibit G's, H's and Exhibit L is incorrect. It appears there may have been some
confusion between exhibits for a 110c and a 112c. Per Rule 1.4.1(6): Please submit the properly labeled
exhibits required under Rule 6.3 for a 110c application.
4) Exhibit I, Rule 6.3.9, is missing.
5) Per Rule 1.4.2(2) (b) (iii) and 1.6.2(1) (b) a signed affidavit of notice posting is required. Please submit the
required affidavit.
6) Per Rule 1.6.2(1) (c): Proof of filing with County Clerk (Exhibit 1) is missing. Please submit proof of filing
with the County Clerk of the application with the noted corrections above included.
Per Rule 1.4.1(8) the applicant has sixty days from the date of this letter, April 5, 2010 to correct the noted issues
in order for the application to be considered complete.
