(v) A hydrologic reclamation plan specifically addressing any potential adverse <br /> hydrologic consequences identified in the probable hydrologic consequences determination prepared based upon <br /> 2.05.6(3)(b)(iv) and shall include preventive and remedial measures as necessary to prevent material damage to <br /> minimize hydrologic impacts, and to meet the performance standards of 4.05. <br /> (vi) The location of each water diversion, collection, conveyance, treatment, <br /> storage, and discharge facility to be used within the permit area. <br /> (vii) A plan for the restoration of the approximate recharge capacity of the permit <br /> area and adjacent area in accordance with 4.05.12(3). <br /> (viii) If the determination of the probable hydrologic consequences (PHC) required by <br /> 2.05.6(3)(b)(iv) indicates that adverse impacts on or off the proposed permit area may occur to the hydrological <br /> balance, or that acid - forming or toxic - forming material is present and my result in the contamination of ground <br /> water or surface water supplies, then information supplemental to that required under 2.05.6(3)(b) shall be provided, <br /> as necessary, to evaluate such PHC and to plan remedial and reclamation activities.
