(Page 2) <br />MINE ID # OR PROSPECTING ID #: _ M-1980-994 <br />INSPECTION DATE: 1.499 1n INSPECTORS INITIALS: RK <br />OBSERVATIONS <br />The Division conducted a pre-operational inspection of the site, after the operator <br />submitted an amended filing of the original amendment application submitted on 6/30/09 on <br />1/12/210. Since the amended filing required the operator to add additional acres, it also <br />required a new notification of adjacent land owners and new Notice in the News Paper. <br />Present during the inspection were Dr. Angela Belantonie the operator's consultant and <br />Mr. Phil Rieg of Asphalt Paving and Redie Mix. <br />All the new permit and amended boundary markers properly posted. Amendment application <br />sign properly posted in two areas given the shape of the permit boundary. One sis located <br />along the access to the site and another to the South west side of the permit area. <br />Angela stated that she is finalizing compensation agreements with manmade structure <br />owners. The commoner period will end at about the end of the month. <br />During the preoperational inspection a small ponded area was noted from GW flow. operator <br />was notified about detaining water for more than 72 hours. Mr. Phil Rieg agreed to allo <br />the water to drain to the natural drainage. He also stated that the operator had water <br />right, but will allow this are to properly drain so no water will be defamed for more <br />than 72 hours. ON 2/5/10. Denise Gonzales of Redi Mix confirmed that the operator had <br />indeed created a small channel so no water is detained in this area. Since the water was <br />noted during the inspection, it will still be sited as a possible PB in this inspection <br />report. <br />I & E Contact Address <br />NAME: MR.
