s <br /> III. COMMENTS-COMPLIANCE <br /> Below are comments on the inspection. The comments include discussion of observations made during the <br /> inspection. Comments also describe any enforcement actions taken during the inspection and the facts or <br /> evidence supporting the enforcement action. <br /> North of the drainage, along the broad ridge between 006-E1 and NE3, a former maintenance <br /> road was observed that was poorly vegetated and rilled along several segments (Photo SL2-27). <br /> Maintenance including installation of water bars, scarification and seeding is warranted. <br /> Channel 006-E2 <br /> E2 is tributary to El. The riprapped channel has a short north fork near its upper end which is <br /> also riprapped (the fork is not depicted on the PMT map). A gully extends up from the upper end <br /> of the riprapped channel fork for approximately 300', and averages 10-12" deep, somewhat <br /> deeper in short segments (Photo SL2-17). A similar gully of approximately the same length <br /> extends up from the upper end of the riprapped main stem channel. Both gullies will need to be <br /> stabilized in accordance with the approved Rill and Gully Plan. Along the lower end of the <br /> main stem of 006-E2, a short distance upstream from an undisturbed shrub patch, there is one <br /> location where evidence of past side-scour, outside of the riprap, is evident(Photo SL2-18). This <br /> area appears to be healing over,but should be visually monitored. <br /> 006 Gulch (Upstream of Ridgeline Access Road) <br /> The regraded "A"Pit highwall slope located to the south of the upper section of 006 Gulch, up- <br /> slope and southeast of the Ridgeline Access Road has been subject to two episodes of instability. <br /> The slope failed following original reclamation grading, resulting in significant displacement <br /> and a primary scarp over 8' in height. Following geotechnical investigation and development of <br /> a remedial plan, the failed slope was graded to a reduced gradient and fully reclaimed in 2004. A <br /> smaller slope failure occurred in the same location in 2008, creating a tension crack extending for <br /> over 200' across the slope, with a scarp 2' to 3' in height in some locations (see Photos SL2-37 <br /> and SL2-38). The operator again regraded the slump location to eliminate the scarp, cracks, and <br /> bulging, and to improve drainage along the road ditch along the toe of the slump subsequent to <br /> SL2 inspection, in the late fall of 2009. Due to the condition of the slope at the time of the <br /> SL-2 inspection, and given the history of instability and uncertainty regarding the <br /> effectiveness of the most recent repair, the Division is excluding the "A" Pit highwall slope <br /> failure area (approximately 9.8 acres) from the SL-2 proposed approval area. <br /> A couple hillslope gullies that have been subject to repair efforts in the past are located on the <br /> slopes of the upper 006 watershed, in the area between the Ridgeline Light Use Road and <br /> Ridgeline Access Road. Down-cutting has reinitiated in these previous repair areas, and <br /> further stabilization measures are warranted (channel shaping and TRM liner installation <br /> 14 <br />
