Attached are a corrected public notice and application from. I'd used the public notice from TR-119
as a base for this notice and forgot that it included the notice of activity within 100' of the highway
right-of-way. This application will not be close at all to the right-of-way so I've removed that portion of
the notice referencing the highway. Sorry for the typo on the application from too. Should have been
a 7 with the 5, not a one! It should be 75 acres. Thanks so much fro the quick review. Please let me
know if there are any other completeness items that I need to address this morning and I'll take care
of them.

I can call TR-120 complete if you replace the general reference to a highway in the public notice with "State Highway
133". Also, please provide a corrected copy of the application form showing the correct disturbed acres (the submitted
copy shows only 15 acres).
