The application shall be submitted within 90 days with all the material in an <br />approvable form within 210 days of the signature date of this Board Order. <br />The Board imposes a Cease and Desist Order against the Operator. The Operator is <br />prohibited from conducting any further activity at the site except for reclamation and <br />environmental maintenance activities until all areas affected by the operation are included in <br />an approved reclamation permit. <br />The Board imposes a civil penalty for 51 days of violation at $100 per day for a total <br />civil penalty of $5,100. All but $1,000 of the penalty is suspended if the Operator complies <br />with the corrective actions in the times specified.
