The Applicant. only submitted one map for Exhibit E•: Rule 6.3.5(1) states the Applicant must <br />include one•map forjhe'min&plan and one map for the Reclamation.Plan;'Please submit 'one*. <br />map for the Mining' Plan and one map for the Reclamation Plan.. The maps must conform to <br />the following criteria:'show the name of the Applicant, must be prepared'and'signed bya- <br />professional engineer, or registered land surveyor, give the date prepared, ide'ntifyand outline <br />the area which corresponds with the application, and show a north arrow. Additional <br />requirements for the Mining Plan Map and the Reclamation Plan Map can be found inthe <br />Construction Materials Rule 6.3.5 (2) and 6.3.5(3). <br />6.3.10 Exhibit J - Proof of.Mailing.of Notices to Board of County Commissioners and Soil' <br />Conservation District <br />6. The Applicant did not submit proof that the permit application was sent to the Board of <br />County Commissioners and the Soil Conservation District as required by Rule• 1.6:2(1)(a)(ii). <br />Proof of notice shall be in the form of a return•receipt of a Certified.mailing or d date <br />stamped receipt`by the appropriate local Board. <br />Your application will.not be considered filed untl,the information listed above is received and <br />found sufficient to begin our review. A decision, date will be established 30 days from the date <br />of receipt of all of the requested information: Additionally, if you have already published notice <br />you will need to republish notice, after the Division considers the application submitted. <br />Republication must occur once within ten (10) days of the submittal date. We,will: notify you <br />when you should initiate'republication of your notice. The final date for receiving comments is <br />ten (10) days after the date of publication or the next regular business day. <br />You have sixty (60).days.from,the`above date to"submit all necessary documents that the Office <br />needs for an application-to be considered filed. If, at the end of the sixty day' period, the <br />application has not been determined to be filed with the Office, the Office shall denythe <br />• '" application and terminate the application file. <br />This letter shall not be construed to mean that there are no other technical deficiencies in <br />your <br />application.
