August 2002 -40- 003 -2203
of each seam to verify that no additional repairs were required. In the event that additional
repairs were needed, Golder marked the location requiring repair on the geomembrane,
documented the location, and brought the repair area to the attention of Poly -Flex. Once
Poly -Flex made repairs, Golder checked the repair location to verify completion. Golder
inspected all seams for quality and completion. Golder personnel conducted a final
walk - through to verify completion of the entire geomembrane system and to identify any
additional damage that may have occurred during construction operations.
Golder recorded observations made during seaming. The fusion - welding summary is presented
in Appendix K -5, and the extrusion - welding summary is presented in Appendix K -6.
6.1.4 Defect and Repair Observations
Golder inspected geomembrane panels for damage that may have occurred during manufacture,
transport, deployment, or installation as discussed in Section 6.1. Golder repeatedly observed
all panels and seams for defects until Ames covered the geomembrane with DCF.
Golder documented defects by assigning a unique defect identification code to each defect, and
marking each defect code on the geomembrane. The summary of detects, repairs, ana
non - destructive testing of repairs, including the approximate location of each repair is presented
in Appendix K -7.
6.1.5 Destructive Testing
Golder selected destructive test locations based on Poly- Flex's completion of approximately
78,661 lineal feet of welding, various working conditions, and on seaming observations
(e.g., suspicion of a defective weld). Poly -Flex obtained and tested 164 fusion seam
destructive samples and 9 extrusion seam destructive samples during the installation of the
geomembrane in the Phase IVa OSA.
•
i.XaoWM'04 %CatRepMDZWm22o3,oaao.a 740t Golder Associates
