§2.05 APPLICATION FOR PERMIT FOR SURFACE OR UNDERGROUND MINING <br />ACTIVITIES --MINIMUM REQUIREMENTS FOR OPERATION AND RECLAMATION <br />PLANS. <br />A public road was re-aligned during the construction of <br />the stockpile. The realignment of this road included <br />the installation of an underground crossing for the rail <br />spur. <br />(vi) Stream fords. <br />There are no stream fords along the haul roads or access <br />roads within the Permit boundary of the train loadout and <br />stockpile areas. <br />(vii) No answer required. <br />(4) Ponds, Impoundments and Diversions <br />(a) General. <br />(i) Each general plan shall: <br />(A) The maps, plans and cross sections for the loadout <br />and stockpile ponds were prepared by or under the <br />direction of and certified by Matthew Sakurada, P.E., a <br />qualified professional engineer with engineering <br />experience in the design and construction of <br />sedimentation ponds, excess spoil and underground <br />development waste facilities. Mr. Sakurada holds a <br />B.S. and M.E. in Civil Engineering and is registered in <br />the State of Colorado, P.E. Number 15836.
