a <br />• 2.03.4(8) <br />(8) Mine Name and MSHA ID Number <br />Mine Name - Hamilton Mine <br />MSHA ID No. 05-04457 <br />(9) None. <br />2.03.5 COMPLIANCE INFORMATION <br />• <br />(1)(a) & (b) <br />The applicant, or any subsidiary, affiliate or <br />persons controlled by the applicant has never; (a) <br />had a Federal or State mining permit for surface <br />coal mining operations suspended or revoked or; (b) <br />forfeited a mining bond or similar security deposit <br />in lieu of bond. <br />(2) Not applicable <br />(3) Listing of Violations - The Operator has received <br />one violation in the past three years while <br />operating the Hamilton Mine. <br />(a) Identifying numbers: NOV C-93-139 was issued <br />by the Division of Minerals and Geology on <br />October 4, 1993. Identifying numbers for the <br />operation are presented on the preceding <br />pages. <br />(b) Description: Failure to republish and <br />redistribute the blasting schedule by mail at <br />least every twelve months. <br />(c) Proceedings: The Operator paid a $350 civil <br />penalty. <br />(d) Current Status: The violation was abated by <br />the Division effective November 23, 1993. <br />(e) Actions: The Operator published a public <br />notice that indicated the blasting operations <br />at the Hamilton Mine had been concluded. <br />(f) Final Resolution: The Division received proof <br />of publication of the public notice and abated <br />the violation. <br />• <br />2.03-7 September 1994
