amen <br />Earth 8 Environmental <br />CQA ACTIVITIES: <br />SUMMARY: <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. <br />AMEC observed placement of structural fill within the Phase 5 VLF fill area. <br />AMEC observed structural fill placement (processed ore) within the Phase IV VLF constantly. Nuclear <br />density testing was performed during placement. <br />AMEC observed the Phase IV geomembrane exposure constantly. <br />AMEC observed construction of the ADR waterline. <br />Testing Summary <br />Laboratory testing continues (Particle Size Distribution, Atterberg Limits, and Moisture- Density <br />Compaction testing) for sampled materials. <br />GENERAL PROJECT ITEMS <br />Summary of Concerns: <br />No concerns <br />Summary of Planning Items: <br />Structural fill placement within the Phase 5 VLF footprint will continue. <br />Continue processing soil liner fill at the South Cresson Clay Processing area. <br />Continue subgrade preparation within the Phase 5 VLF footprint. <br />Continue construction of the ADR and Crusher waterlines. <br />Continue exposure of the Phase IV VLF geomembrane for re- certification. <br />Summary of Non - conformances: <br />None Issued <br />Summary of Meetings and Discussions: <br />CC& V. <br />CC &V held a Weekly Construction meeting on October 27, 2009. Items discussed included Safety, <br />Environmental, Schedule, QC documentation, and CQA. <br />Ames Construction: <br />Daily Safety /Tailgate Meetings: AMEC personnel attended the Ames daily safety meeting held prior to <br />the start of shift. Discussions centered on safety and planned work activities. <br />AMEC: <br />Spoke with John Lupo concerning the design issues with the Crushing /Sediment pond culvert (42 ") and <br />the recertification at the old LOB site.
