Inc. <br />Permit No. M-2006-039 North Clear Creek Construction Materials Mine <br />Garfield County Colorado <br />Dear Mr. Dupler, <br />In accordance with the regulations of the Division of Reclamation, Mining and Safety, Chevron <br />U.S.A. inc. hereby files its annual report and annual fee for the subject mine. <br />Preceding Year <br />The North Clear Creek Construction Materials Mine was opened in March of 2008 and the mine <br />has been in operation since that time. From November 2008 to end of October 2009, a total of <br />120,314 tons or 162,424 cubic yards of material has been removed. Reclamation and <br />revegetation activities have been initiated on top soil piles and slopes. A noxious weed control <br />program has been in place since inception and storm water management controls have been in <br />place and are monitored regularly. The mine was inspected by Travis Marshall on October 10, <br />2008 and passed inspection. <br />A check in the amount of $323.00 for the annual fee, along with a survey map of the current <br />mine showing the disturbed areas are enclosed.
