(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1996-052 <br />INSPECTION DATE: 11/17/09 INSPECTOR'S INITIALS: ECS <br />OBSERVATIONS <br />This was a routine inspection for the Firestone Gravel Resource, a 112c sand and gravel permit operated by <br />L.G. Everist (LGE), conducted by Eric Scott of DBMS. DRMS visited the site with Lynn Mayer-Shults of <br />LGE. This site has been in Temporary Cessation (TC) since February of 2006, and the site was transferred <br />from Ready Mixed Concrete to LGE in July of 2008. <br />Because the site has been in TC for the last 4 years, there has been no recent mining activity. There are several <br />stockpiles of material and topsoil located on site, and the excavation has filled with water. LGE will need to <br />confirm that there is an adequate substitute water supply plan for any exposed groundwater at the site while in <br />TC or during future operations. DRMS was unable to confirm a SWSP for the site due to an ongoing electronic <br />records retrieval issue during the writing of this report. <br />The current bond posted for the site ($49,450) is inadequate to reclaim the site according to the existing <br />reclamation plan. However, LGE stated during the inspection that an amendment for this site to increase the <br />permitted acreage and revise the mining and reclamation plans would be submitted within 60 days. Therefore, <br />it seems reasonable at this time to review the permit amendment and adjust the bond accordingly at that time. <br />No other issues were noted during this inspection.
