(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1977-022 <br />INSPECTION DATE: 1 1 /4/2009 <br />OBSERVATIONS <br />INSPECTOR'S INITIALS THM <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division of <br />Reclamation, Mining and Safety. <br />The mine identification sign was not visible at the entrance of the mine. The sign was lying on the ground <br />approximately 30 feet from the mine entrance. Per Rule 3.1.12, a mine identification sign shall be posted at the <br />entrance of the mine site and be clearly visible.
