(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-9nnn-O51? <br />INSPECTION DATE: - 0.493/02 INSPECTORS INITIALS: -Sly <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site as part of sites to be <br />inspected during the fiscal years 2009-2010. The site is a 193.28 acres, phased 112 sand <br />and gravel operation, located in Fremont County East of Florence, Co along the north side <br />of Highway 120. <br />Present during the inspection was Mr. Tom Smith and Mr. Richard Lisgar of Rocky Mountain <br />Materials. <br />Permit sign was properly posted at the access to the property. <br />The site was not active at the time of the inspection. <br />Along the west side of the small pit area of 3 acres is an active oil pump and storage <br />tank that is included in the permit area. A damage agreement with the owner of the well <br />was submitted when the reclamation permit was approved in 2000. <br />As stated above the site was not active and the total affected area of the pit and <br />associated disturbances appeared to be less than 4 acres. According to Tom, the site is <br />only utilized on as need basis and does not expect the disturbance of the pit area to <br />increase that much during the next year. <br />Pit high walls were stable. The measure approximately 4 feet in height extending North to <br />south approximately 350 ft long. <br />The current warranty in the amount of $32,400.00 is more than adequate to cover the cost <br />of reclamation. Operator is aware that if the current affected acres exceed the maximum <br />phased area allowed under the permit, to submit a Technical Revision to increase the <br />affected acres. The permit allows the operator to affect all of the 193.28 acres as long <br />as the appropriate warranty is posted and accepted by DRMS. <br />The operation is in compliance with the approved reclamation permit. <br />I & E Contact Address <br />NAME: MR.
