ORIGINAL <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: m-19Rn-9aa <br />INSPECTION DATE: ] n.?gAln9 <br />OBSERVATIONS <br />PUBLIC <br />INSPECTOROS INITIALS: RK <br />FILE <br />The Division conducted a fourth quarter GW and under drain discharge sampling of the site <br />on 10/26/09. The area had received approximately 7 inches of snow the night before. <br />Present during the under drain sampling and valley leach area inspection was Mr. Mark <br />Vannoni. Split samples for wad cyanide analysis were collected from both the south under <br />drain and 4 inch pipes. After the sampling, the valley leach facility was inspected. <br />The following readings were recorded from the High and Low Volume solution collection <br />system transducers. <br />Phase I High Volume Solution Collection transducer # 1 was recorded at 44.0 # 2 was at <br />44.3 and # 3 was at 442.0 Piezometer for the pond was at 47.7. Low volume solution <br />collection transducer # 1 was at .24 and # 2 was at .46. <br />Phase II and III High Volume Solution Collection system transducer #4 was recorded at45.9 <br />45 was at 44.7 and #6 was at 45.6, piezometer for the pond was at 47.7 feet. Low volume <br />solution collection transducer # 1 was recorded at .20 and # 2 was at .45 <br />Phase IV high volume solution collection transducer # 7 was recorded at 23.2 # 8 was at <br />22.8 and # 9 was at 22.7. Low volume solution collection transducer 3 1 was recorded at <br />15.2 and # 2 was at 10.1 inches of hydrostatic head. ( ALL other transducer readings are <br />in feet of hydrostatic head) <br />Inspected the external pond, it was holding a partially frozen water. Low volume <br />transducer # 1 for the pond was recorded at 13.3 and # 2 was at 11.2 inches of hydro <br />static head. <br />Present during the GW sampling in Arequa Gulch was Mr. Lawrence Myers. After removing <br />three casing volumes from the GW monitoring wells CRMW 3A AND 3B, A SPLIT for Sulfate and <br />Wad Cyanide were collected, with the operator collecting a split sample. <br />Samples were placed in ice and delivered to the Colorado Health Department Lab the same <br />day. <br />I & E Contact Address <br />NAME: MR.
