amec` <br />Earth 8 Environmental <br />was performed using a CAT 375 excavator. This was performed to re-certify the liner in this area. <br />Damage to the geomembrane was observed near the anchor trench and has been documented. An <br />AMEC representative was present at all times during the exposure. It is anticipated liner repairs and <br />re-certifying the Phase IV liner will commence in November, weather permitting. <br />CQA ACTIVITIES: <br />SUMMARY: <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. <br />AMEC observed placement of structural fill within the Phase 5 VLF fill area. <br />AMEC observed structural fill placement (processed ore) within the Phase IV VLF constantly. Nuclear <br />density testing was performed during placement. <br />AMEC observed and accepted subgrade preparation within the Phase 5 fill area. <br />AMEC observed the Phase IV geomembrane exposure constantly. <br />AMEC observed excavation and transport of topsoil. Topsoil removal within the Phase 5 VLF was <br />completed on the 15th of October 2009. <br />Testing Summary <br />Laboratory testing continues (Particle Size Distribution, Atterberg Limits, and Moisture-Density <br />Compaction testing) for sampled materials. <br />GENERAL PROJECT ITEMS <br />Summary of Concerns: <br />No concerns <br />Summary of Planning Items: <br />Structural fill placement within the Phase 5 VLF footprint will continue. <br />Continue processing soil liner fill at the South Cresson Clay Processing area. <br />Continue subgrade preparation within the Phase 5 VLF footprint. <br />Continue construction of the ADR and Crusher waterlines. <br />Continue exposure of the Phase IV VLF geomembrane for re-certification. <br />Summary of Non-conformances: <br />None Issued <br />Summary of Meetings and Discussions: <br />CC& V.• <br />CC&V held a Weekly Construction meeting on October 13, 2009.
