Sampling and Analysis Plan - Operations & Post-Operations
• Information recorded on the Surface-Water Monitoring Form will not necessarily be repeated in
the bound field logbook. The bound field logbook will refer to the sampling form when
necessary and vice versa.
5.1.3 Groundwater Monitoring Form
A Groundwater Monitoring Form (see Appendix B) will be used for each individual site sampled.
This form allows for expedited documentation of the following information:
• Sample identification
• Sample date
• Sample site location description and sketch
• Sampling personnel
• Equipment identification
• Field equipment calibration information
•
• Static water level
• Bore volume
• Total volume purged from well
• Measured flow rates
• Number and type of filters used
• Field parameters measured and results
• Other pertinent observations (gas meter readings)
Information recorded on the Groundwater Monitoring Form will not necessarily be repeated in
the bound field logbook. The bound field logbook will refer to the sampling form when
necessary and vice versa.
