Sampling and Analysis Plan - Operations & Post-Operations <br />• 5.0 SAMPLE CUSTODY <br />The data collected during the sampling process require that the possession of samples be traceable <br />from the time they were collected, through the laboratory analytical process. This section <br />describes procedures to effectively track sample custody. <br />5.1 Sample Documentation <br />In order to provide adequate information of the sampling event, the field sampling crew will <br />maintain a bound field logbook and Surface-Water and Groundwater Monitoring Forms (see <br />Appendix B). These field documents will contain sufficient details that describe daily site <br />conditions, sample site identifications and descriptions, recorded field parameters, and site <br />personnel. Entries will be made in indelible ink, with all corrections consisting of initialed line- <br />out deletions. A sample label will be completed and adhered to each sample bottle or container. <br />The documents to be completed for each sample generated during the sampling program are: <br />• Bound field logbook <br />• <br />• Surface-Water or Groundwater Monitoring Form (includes site description, sample <br />location, recorded field parameters) <br />• Sample tag and/or label <br />• Chain-of-custody transmittal form <br />Upon completion of sampling, a chain-of-custody transmittal form (see Appendix B) will be <br />filled out, and samples will be shipped by way of overnight carrier to the analytical laboratory. <br />5.1.1 Bound Field Logbook <br />All pertinent field survey and sampling information will be recorded in a bound field logbook, <br />during each day of the field sampling and at each sample site. All entries will be made in <br />indelible ink. Each day's entries will be initialed and dated by the field sampling leader. All <br />corrections shall consist of initialed line-out deletions. <br />At a minimum, entries in the bound field logbook shall include: <br />American Shale Oil, LLC 10
