(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-19Rn-944 <br />INSPECTION DATE: 911 A-Ing INSPECTORLIS INITIALS: _ RK <br />OBSERVATIONS <br />The Division conducted a Low volume solution collection booster pump placement inspection <br />on 10/1 and on 10/6 while in the area, verification inspection of the Low Volume <br />transducer readings. Present during the site visit were mark Vanoni and Jeff Campbell. <br />When the monthly inspection was conducted on 9/17/09, DRMS had cited a possible problem <br />to the general mine plan for not maintaining the hydrostatic head in the double lined <br />system, for Phase IV LVSC PUMP # 1 TRANSDUCER BELOW TWO FEET OF HYDROSTATIC, AS THE <br />PERMIT CONDITION REQUIRED. In the past due to the same problems, CC&V had replaced pumps <br />with additional capacity pumping including the placement of a Variable Frequency Drive <br />pumps, apparently even with a maximum capacity pumping rate of over 100 gpm, the level <br />exceeded the two feet of hydrostatic head. <br />DRMS, met with upper management and informed them that permanent fix will have to be in <br />place no later than 11/17/09. If after the permanent fix is in place and there is still a <br />sustained exceedance of the 2 feet of hydrostatic head in the double lined system, DRMS <br />will send a reason to believe a Violation Exist letter and bring the item before the MLRD <br />BOARD. <br />CC&V agreed to temporarily place an auxiliary booster pump. DRMS observed the placement <br />of the booster pump which was completed on 10/1/09 @ 4:30 p.m. After the completion of <br />the Fremont County site, drove to CC&V and verified LVSC transducer readings for Phase IV <br />Low volume pumps which were at 15.1 for transducer # 1 and 10.4 for # 2. <br />After collecting the transducer readings, met with Mr. Kevin Reilly, Vice President and <br />in charge of the Valley Leach Facility about the status of the replacement pumps During <br />the meeting, Jeff Campbell and Mark Vanoni of the environmental group were also present. <br />Kevin provided the type of pumps that CC&V will order to accommodate the flow and prior <br />to installation, CC&V will make sure the pumps will go smoothly and reach the low volume <br />solution collection sumps which are close to 400 feet below surface.(lateral depth, not <br />perpendicular). In the interim, it was agreed to keep the High Volume pregnant ponds at <br />the level they were observed at on 10/6/09.
