(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-9009-07 <br />INSPECTION DATE: 2/99 m9 INSPECTOR?S INITIALS: RK- <br />OBSERVATIONS <br />The Division conducted a pre-operational inspection of the 110 permit while in the area. <br />Normally the does not conduct a pre-operation inspection of 110 permits. However, since <br />the Division was going to be in the area, and the maps were not clear enough to delineate <br />the pre- law disturbances and # of old trailers and structures still in place, DBMS <br />conducted the inspection. <br />Operators consultant, Mr. Steve Obrien, was notified of the pending inspection but was <br />unable to attend. <br />The proposed access road gate was open and since there was no posted No Trespassing sign <br />posted, drove utilizing the access road along 33 lane. The site for the most part was as <br />described in the proposed permit application. No permit boundary permit markers were in <br />place. There were large and small stockpiles of processed material noted. Some appeared <br />fresh. There was a small screen located within the previously disturbed are pit floor. <br />There was numerous quantities of none mine debris stockpiled along what appeared to be <br />the old pt floor. <br />Along the North west side of the access road a small are of wet lands were noted. The <br />presence of these wet land areas were not discussed in the narrative portion of the <br />application. Given its proximity to the pit area, there is a very good likelihood the <br />mine could drain the source of GW for the wet lands. Operator will need to address the <br />probable impact to the wet lands from the proposed mining operation. <br />The Division contacted Mr. Steve Obrien the consultant about the findings and Mr. Obrien <br />agreed to address.
