This report is filed in accordance with Rule 2.02.6(3)(b) of the Regulations of the
Colorado Mined Land Reclamation Board for Coal Mining, adopted in accordance with
the Colorado Surface Coal Mining Act, C.R.S. 43-33-101 et seq. This reportis to be
submitted within 30 days after the end of the period of exploration specified in the
approved Notice of Intent, as amended.
Attach a map showing:
a. The final locations of all holes drilled during this project;
b. The location of all roads modified or constructed for the project; and
c. All areas significantly disturbed by the operator.
2. Have all disturbed areas, including drill holes, been reclaimed in accordance with
the approved plan and the above-referenced regulations? Yes
3. Were any newly constructed projects roads retained for the landowner? No
If yes, a) attach a letter from the landowner that grants consent, b) indicate the
road location on a map, and c)describe the approximate specifications of the road
(width, length, surface material used).
4. Did any drill holes experience surface artesian flow conditions? No
If yes, locate these holes on a map and describe sealing and plugging procedures
that were used.
5. Were any drill holes converted to a water well? No If yes, list the State
Engineer's Well Permit Number and name and address of the user.
6. Does the prospector wish this report to be considered confidential? (See Rule
2.02.7(s)). No If yes, attach a justification for the request.
