(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1977-911 <br />INSPECTION DATE: 9.11549 INSPECTORS INITIALS: R_ <br />OBSERVATIONS <br />The Division conducted a site visit of the quarry face after receiving a call from the <br />operator regarding another slope failure on site. The site is under a Board cease and <br />desist order. DRMS visited the site to verify the fact that the LICA slope monitoring <br />system on 9/11/09. The system was operational at the time of the slope failure on 9/13/09 <br />as was the case on 9/11/09. <br />With Colorado Geological survey's Geological Engineers, T.C Wait and Jon White present, <br />the failure surface was observed from close up. Massive displacement more than was noted <br />during the last failure in early August was observed. These types of failures were <br />expected and will more than likely happen again until the operator submit a revision by <br />the end of May to permanently repair the slopes so such occurrence would be at best <br />minimum given the geological location of the area. In addition to CGS personnel, the <br />operator's own outside consultants who are experts in land slide were also present on <br />site evaluating the slide area. The consultant's recommendation in addition to other <br />engineering studies being conducted will be implemented when the Revision to the permit <br />is submitted by the end of May or sooner. The Revision will focus on achieving post <br />mining stable slopes rather than mining to achieve the desired factor of safety for the <br />reclaimed slopes and will be reviewed by CGS, personnel when submitted.
