(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn-1981-98A_ <br />INSPECTION DATE: 9./1 am9 INSPECTOR=S INITIALS: MAC <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Colorado Division of Reclamation, Mining and <br />Safety (Division). Greg Nelson of Weld County Public Works Department was also present for the inspection. <br />The Thomas Gravel Pit 4 is located approximately 4.5 miles west of Keota, Colorado. The site is permitted for <br />87.60 acres and the approved post mining land use is general agriculture. <br />The permit sign was posted at the entrance to the site and the boundaries of the affected area were clearly <br />delineated per Rule 3.1.12. Mining is no longer taking place at this site. During active mining operations, <br />mining was progressing westward into a hillside. The mine face has been graded to 4H:1 V or flatter. The <br />northeast portion of the mine, approximately 10 acres, was the final area to be mined. Topsoil was replaced and <br />seeded in this area in 2007. The vegetation is beginning to establish, but will require more time before it is in a <br />releasable condition. The remainder of the disturbed area was seeded in 2004; the vegetation in this portion of <br />the mine is well established. <br />The access road that runs through the center of the site has not been reclaimed. The Operator must reclaim the <br />access road in accordance with the approved Reclamation Plan. This includes replacing topsoil and seeding the <br />access road within the permit boundary. If the landowner wants the road to remain as a service road, then the <br />Operator will need to submit a signed and notarized statement from the landowner indicating that the road is to <br />remain. A pile of concrete was noted near the southwest corner of the permit boundary. Mr. Nelson stated that <br />the concrete was not related to the mining operations and that it was most likely illegally dumped on the site. <br />The Operator is responsible for the concrete pile. If it is not to be used for reclamation, it must be removed from <br />the site. This has been cited as a problem and will require corrective action by the Operator.
