(Page 2) <br />MINE ID # OR PROSPECTING ID #: NA-1989-15.5 <br />INSPECTION DATE: 2.L98m9 <br />INSPECTORS INITIALS: - K <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site as part of sites to be <br />inspected during the fiscal years 2009/2010. The site is a 717 acre permit located <br />approximatelyl.5 miles west of Fountain, Colorado. The maximum affected acres is set 277 <br />acres at any given time. The site was active at the time of the inspection. The current <br />warranty in the amount of $310,388 was deemed adequate to cover the current affected area <br />reclamation. <br />Mr. Travis Haggard was operating a front end loader feeding the sand screen and loading <br />trucks at the time of the inspection. motor grader and a haul truck were located along <br />the NW west side of the pit area. Active mining was talking place along the east side of <br />the affected pit area. Pit high walls extend east to west along the affected area. High <br />walls were estimated to be a approximately 15 feet in height. High walls were stable, no <br />major sign of failure surface was noted, given the alluvium nature of the material being <br />mined. <br />Numerous processed stockpiled sand, some old some new were noted along the north side of <br />the active mining area. Top soil overburden stockpiles located along the North West side <br />of the permit area were found to be in good repair. The old wash pond along the same area <br />was dry and almost full. <br />The small fresh water pond along the north side of the main pit area was full, a water <br />tank was hoisted above the pond for easy access to fill up the water trucks when needed. <br />The recent rains the area received was reflected by a small ponded water along the south <br />eastern side of the active mining area. The entire site was driven and inspected. Permit <br />sign is properly posted at the access to the property.
