Dan Collins, Co-Trustee, Deep Creek Placer
1417 S. Grandview Dr.
Tempe, AZ 85281
Sept. 1, 2009
Division of Reclamation, Mining, and Safety
1313 Sherman St., Room 215
Denver, Colorado 80203
Annual Report: Deep Creek Placer (Permit No. M-1986-102)
Dear Sir or Madam: Please accept this Annual Report on the "Deep Creek Placer," a temporarily
inactive limestone mine 7 miles west of Telluride, Colorado-at the-confluence-of Deep Creek and
the San Miguel river.
--No mining has taken place in the year Sept. 1, 2008 - present
--Portals to the mine have been checked and they are secure
--Equipment room entrance locked and secured.
--Routine maintenance of access road carried out, summer 2009
--Annual maintenance of stormwater "ponds" along access road for capturing runoff has been
carried out.
--Access road gate is secure and locked
--Bond continues to be sufficient for reclamation (if necessary) of mine site.
--No disturbances to the surrounding land or the mine itself are anticipated.
--Regular maintenance of roads, equipment apron, portals, and stormwater ponds will be
conducted by myself over the course of the coming year.
The mine is part of a property owned by the Collins Living Trust. I am co-trustee of the Trust
along with my two sisters, Jean Collins Norris (Chatham, Massachusetts) and Lois Morehead
(Chico, California). Out mother, Kathryn O. Collins, passed away January 10, 2004.
While there are on-going discussions in the family regarding future uses of the mine, the cost of
fuel and other factors such as the demand for limestone in the marketplace make it difficult to
predict what the future holds for the property. We are requesting that the mine continue as an
"inactive" but permitted entity during the coming year.
Respectfully submitted,
Dan Collins
Cell: 480-206-2037
Email: dan.collins@asu.edu
Attachments: annual fee (check for $323.00), report request, associated map of permitted area.
