O <br />O <br />W <br />Evergreen Analytical, Inc. <br />Date: 12-Aug49 <br />Lab Order: 09-5457 <br />Client Project ID Union Milling Co <br />SAMPLE RECEIVING <br />CASE NARRATIVE <br />Sample(s) were hand delivered to the laboratory by the client. <br />Custody seals were not present. <br />The temperature of the sample(s) upon arrival was 25.4°C. <br />Sample(s) were received in good condition and in the proper container. <br />The Mercury sample was received out of holding times. JD/TP <br />QUALITY ASSURANCE (QA) <br />Analyses performed on samples in this work order by EAL meet the requirements of the EAL Quality <br />Assurance Program unless otherwise explained. Analyses of RCRA samples meet the requirements of <br />NELAC and Utah Rule R444-14 unless otherwise explained. TP <br />CLIENT SERVICES <br />There are no anomalies to report. SG <br />METALS ANALYSIS <br />Method TCLPHG: The Mercury sample was received and analyzed outside the method specified <br />holding time (H). There are no other anomalies to reprt. MB/TP <br />Method TCLPMETAL: The Arsenic RPDs for the matrix spike and matrix spike duplicate (MS/MSD; <br />on another clients samples) recoveries are above the QC limits, but the spiked recoveries are within QC <br />limits. There are no other anomalies to report. TP
