Preliminary <br />review of the information received determined that the following items must be received before the Division can consider <br />your application as being submitted and technical review can begin: <br />APPLICATION FEE: The Division did not receive the $2,696.00 application fee with the application package. <br />This fee must be received before your application can be considered complete and technical review can begin. <br />APPLICATION FORM: It was noted that the incorrect application form was used for the Pierce application. <br />A 112d form was submitted (for a Designated Mining Operation). Sand and gravel pits are not considered <br />DMOs therefore a regular 112c construction materials application form should be used in this case. <br />The remainder of this letter may be used by you to double check the adequacy of your application exhibits before <br />it is re-submitted, as it outlines all the items and criteria that will be reviewed during the "technical review" phase <br />of the application. <br />EXHIBIT A - Legal Description (Rule 6.4.1): The legal description must identify the affected land and be <br />wholly adequate for field location of the property. It can be in the form of metes and bounds survey or a <br />description by reference to township, range and section to at least the nearest quarter-quarter section. Where <br />applicable, the street address or lot numbers may be used. <br />EXHIBIT B - Index Map (Rule 6.4.2): An index map showing the regional location of the affected land and all <br />roads and other access to the area. A standard U.S. Geological Survey topographic quadrangle or equivalent is <br />acceptable. Scale criteria need not be followed for this map. <br />EXHIBIT C - Pre-mining and Mining Plan Map(s) of Affected Lands (Rule 6.4.3): One or more maps may <br />be necessary to legibly portray the following information: <br />(a) all immediately adjacent surface owners of record; <br />Office of Office of <br />Mined Land Reclamation Denver • Grand Junction • Durango Active and Inactive Mines
