(Page 4) <br />MINE ID # OR PROSPECTING ID #: M-1977-024 <br />INSPECTION DATE: 7/93/119 INSPECTOR=S INITIALS: MAC' <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when considered <br />by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />1. Concrete is being stored at the site. The materials stored on the site are not noted in the <br />approved mining and reclamation plan. <br />Corrective actions are to: <br />The Operator will need to submit a Technical Revision describing the materials stored on site <br />and how they will be used or reclaimed. The Technical Revision shall be submitted on or <br />before September 4, 2009. <br />12. Sediment is being transported outside of the affected area through gullies and the natural <br />drainage channel. The Operator has failed to protect areas outside of the affected land <br />from damage and has failed to stabilize all surface areas of the affected land to <br />effectively control erosion. Rule 3.1.6(3) states that all surface areas of affected land shall <br />be stabilized and protected so as to effectively control erosion. Rule 3.1.5(3) states that all <br />grading shall be done in a manner to control erosion and siltation of the affected lands, to <br />protect areas outside of the affected land from slides and other damage. <br />Corrective actions are to: , <br />The Operator will need to implement erosion control measures such as straw bales, silt fences, <br />or erosion-control blankets. The Operator shall submit a Technical Revision describing how the <br />gullies will be repaired and protected from further erosion, including plans for reseeding any <br />areas that will be graded.
