(Page 2) <br />MINE ID # OR PROSPECTING ID #: N4-1980-944 <br />INSPECTION DATE: 8114109 INSPECTOR=S INITIALS: RK <br />OBSERVATIONS <br />The Division conducted a monthly monitoring inspection of the Valley Leach Facility s ON 8/4/09. Present during the <br />inspection was Mr. Mark Vanoni of CC&V. <br />The inspection began around the Phase IV valley leach facility area. The access road drainage leading to the High and low <br />volume solution collection area appear to have reported to the edge of the liner along the south side. Mr. Vanoni was <br />informed to correct the problem immediately and prior to my departure of the site the problem was resolved. <br />While in the area, collected the following readings from the High and Low Volume solution collection transducers. <br />High Volume Solution Collection Transducers. # 7 was recorded at 10.2 #8 was at 9.7 # 10.2 and # 10 10.6. <br />Low volume solution collection transducer # 1 was 18.6 # 2 was at 9.1 inches of hydrostatic head. <br />Inspected the Phase I and II & III valley leach facility and recorded the following readings from the High and Low volume <br />solution collection transducer. <br />Phase I High Volume Solution Collection transducer # 1 was recorded at 20.7 # 2 was at 19.7 and # 3 was at 19.9. Low <br />Volume # 1 was at.35 and # 2 was at.40 <br />Phase H & III, High Volume Solution collection transducer #4 was recorded at 12.6 # 5 was at 12.1 and # 6 was at 12.2. Low <br />volume # 1 was at.20 and # 2 was at.50. <br />Inspected the External pond it was holding some water. The read out meter for the low volume solution collection <br />transducer #1 was not functioning and # 2 was at 11.3 . The read out meter for # 1 according Mr. Vanoni the read out <br />meter is supposed to be replaced as soon as possible. <br />Discussed briefly about the verification of transducer readings for the pregnant pond with the piezometer readings. It was <br />decided the best time to shut down the pumps to verify readings would be sometime in October after the summer rainy <br />season is over. <br />All surface water best management practices for the site with the exception of the item mentioned above were found to be <br />in good repair. <br />Inspected the under drain area. All lower ponds with the exception of the two lower ponds were dry. The two lower ponds <br />were holding very small amount of water.
