ORIGINAL <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1980-gad <br />INSPECTION DATE: 7_193409 INSPECTORS INITIALS: RK <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the newly constructed load out bin area <br />for which the Division is in the process of reviewing the as built certification report <br />provided by the operator on 7/17/09. <br />As approved in amendment # 9 CC&V had proposed to move the LOB to its new location. The <br />only as build certification that is required for the LOB is the portion of the structure <br />that is within the certified liner and connected to the valley leach facility. The rest <br />of the LOB including the transfer station do not require as build certifications. <br />Present during the inspection was Mr. Tim Comer Environmental Manager for the site. <br />The entire LOB area which includes the concrete walls that will be utilized by the haul <br />trucks was inspected. The concrete walls and associated structures were in place. No <br />displacement of liner was noted. A few big rocks were noted pushed along the valley leach <br />facility along the west side. Mr. Comer was notified to remove these rocks from the area, <br />given their close proximity to the lined facility. <br />The area was inspected before during the grading and liner deployment operations, as <br />required, to verify QA/QC. In addition the Division receives the weekly construction <br />reports for or records. The facility appeared to have been constructed as approved and as <br />indicated in the PE certified report submitted to DRMS. <br />Construction of other facilities as approved in Amendment # 9 is continuing. The old Rubi <br />leach pad is almost removed in anticipation of the expansion of the Mina Cresson mine in <br />the area. Material from the old reclaimed pad is used to backfill the pits. Most of the <br />mining in Phase IV Altman pit is expected to be completed within the next month. <br />Looked at the reclamation success in the recmtly seeded visual beran along Southfield, it <br />looked stable and vegetated. <br />Discussed the future construction plans as approved in amendment # 9. <br />1) CC&V plans to submit a Technical Revision to increase the height of the leach pad <br />from the current approved height of 10,600 feet to approximately 10,800 feet WITH <br />OUT INCREASING THE FOOT PRINT OF THE STORAGE AREAS. . <br />2) Increase the capacity of the existing auxiliary carbon columns. DRMS suggested both <br />items to be submitted as one TR. <br />CC&V plans to submit the TR as soon as plans are finalized. Both items revising the <br />current approved plan will require adjusting the warranty.
