(Page 2) <br />MINE ID # OR PROSPECTING ID M-1990-116 <br />INSPECTION DATE: 7/23/09 INSPECTOR'S INITIALS: FCS- <br />OBSERVATIONS <br />This was a routine monitoring inspection for an active 110c sand and gravel operation conducted by Eric Scott <br />of DRMS for the Phillips County Kennedy Pit site. DRMS toured the site with Phillips County representative <br />Randy Schafer at approximately 1105. <br />The required site entrance postings were present, but the permit number was illegible, so the sign will need to <br />be repaired or replaced. Site boundary markers were not present as the mine is to be defined by the pre-existing <br />stream channel. The mine appeared to be largely in compliance at the time of the inspection. However; the <br />operator will need to submit an affidavit for inert fill and other information as specified in 3.1.5(9) of the Rule <br />for the concrete slabs that have been hauled to the site. This site did not have any potential violations at this <br />time, although the condition of the site in general was a bit rough. Several weed species that should be <br />eliminated, such as thistle, were noted, and there were the beginnings of some erosion issues, as well as several <br />areas of steep (vertical) slopes remaining were mining activities had been completed. <br />The Division would like to encourage that concurrent reclamation activities be conducted wherever possible, <br />and that sites be maintained in good order regardless of intermittent use patterns.
